Common OWCP Forms for Federal Employees | Kansas City OWCP Doctors

Plain-English explanations of CA-1, CA-2, CA-7, CA-16, CA-17 and CA-20, with links to the official Department of Labor versions of each form.

Different OWCP forms serve different purposes. Some report an injury, some request wage-loss compensation, and some are completed by the treating physician to describe diagnosis, findings, and work capacity. Using the appropriate OWCP form and submitting complete medical documentation can help reduce unnecessary delays and requests for additional claim development. Every form below links directly to the official Department of Labor source so you are reading the current revision.

What Our Providers Do — and What OWCP Decides

Restore Muscle and Joint provides medical evaluation, treatment, and documentation for federal employees receiving care under the Federal Employees' Compensation Act, or FECA. Our providers document diagnoses, objective clinical findings, treatment recommendations, functional limitations, work restrictions, and medical causation opinions when supported by the examination and available medical evidence.

Our clinical team can complete the healthcare-provider portions of applicable OWCP forms related to a patient's medical care. Decisions regarding claim acceptance, compensation, benefits, medical authorization, and eligibility are made by the U.S. Department of Labor and other applicable federal authorities.

Our team can explain the general purpose of common OWCP forms and complete healthcare-provider portions of forms that relate to your medical treatment. We do not act as a legal representative, claims representative, or benefits representative, and we do not make filing or eligibility determinations.

For patients whose claims have been denied or require additional development, our providers can evaluate whether additional medical examination, objective findings, diagnostic information, or medical-opinion documentation may be clinically appropriate.

Billing: for covered and authorized services related to an accepted FECA condition, Restore Muscle and Joint may bill the appropriate OWCP program directly. Coverage, authorization, and eligibility are determined by OWCP and remain subject to applicable program requirements.

CA-1 vs. CA-2: Understanding the Difference

The distinction between these two forms is generally about timing rather than severity. Final claim classification and adjudication are handled by OWCP under its own rules.

    CA-1 — Traumatic Injury

Generally associated with an injury occurring during a single workday or shift, such as a fall, a lifting injury, a vehicle collision, or an assault. Continuation of Pay provisions under FECA apply to traumatic injury claims, subject to OWCP requirements.

    CA-2 — Occupational Disease

Generally associated with a condition that develops over more than one workday or shift, such as carpal tunnel from repetitive scanning, shoulder tendinitis from repeated lifting, or degenerative knee and back conditions related to years of walking a route.

OWCP evaluates medical evidence when determining whether a diagnosed condition is causally related to federal employment. When medically supported, a physician's report should identify the diagnosis and explain the medical relationship between the diagnosed condition and the claimed work factors. Detailed guidance is published by the Department of Labor in the FECA Procedure Manual.

A General Overview of the Claim Process

    1. Report the injury to your supervisor

Reporting in writing and keeping a copy creates a clear record. In qualifying traumatic injury cases, the employing agency may issue a Form CA-16 authorizing initial examination and treatment.

    2. Seek a medical evaluation

Early, objective clinical documentation contributes to a complete medical record. FECA generally allows an injured federal employee to select an initial treating physician, subject to applicable OWCP requirements.

    3. Submit the CA-1 or CA-2 through ECOMP

Your agency routes the claim to the Department of Labor. Keep your case number once it is assigned.

    4. Provide the medical evidence

This typically includes the CA-20 attending physician's report, the CA-17 duty status report, and treatment notes describing diagnosis, objective findings, and the physician's causation opinion where supported. Our providers complete the healthcare-provider portions of these documents.

    5. Wage loss and travel reimbursement

A CA-7 is used to claim compensation, with a CA-7a for intermittent time and a CA-7b for leave buy back. OWCP-957A and 957B are used for travel reimbursement requests.

    Frequently Asked Questions

Where can I find the official OWCP forms?

Every form on this page links directly to the Department of Labor at dol.gov so you are viewing the current revision. Most claims are then submitted electronically through ECOMP at ecomp.dol.gov.

What is the difference between a CA-1 and a CA-2?

A CA-1 is generally associated with a traumatic injury occurring during a single workday or shift, such as a fall or a lifting injury. A CA-2 is generally associated with an occupational disease or condition that develops over more than one workday or shift, such as carpal tunnel or tendinitis. Continuation of Pay provisions apply to traumatic injury claims. Final classification and adjudication are handled by OWCP.

    What is a CA-16?

Form CA-16, Authorization for Examination and/or Treatment, is issued by the employing agency. For qualifying traumatic injuries, an agency may issue a CA-16 to authorize an initial medical examination and treatment. A properly issued CA-16 may provide authorization for initial treatment for a limited period, subject to OWCP rules and any subsequent determinations regarding the claim. Agencies commonly issue it through ECOMP.

Who completes the CA-17 duty status report?

The treating physician completes the medical portion describing current work capacity, and the employing agency completes the job requirements portion. Our providers document measurable restrictions — lifting limits, hours, and postural tolerances — based on the clinical examination so the agency can review available duty options.

What is the CA-20 attending physician's report?

The CA-20 is where the treating physician records the diagnosis, objective findings, period of disability, and, when medically supported, an opinion on the relationship between the diagnosed condition and the claimed work factors. OWCP reviews this medical evidence during claim development.

    When is a CA-7 used?

A CA-7 is used to claim compensation for wage loss, generally after Continuation of Pay ends or when COP does not apply, and to claim a schedule award for permanent impairment after maximum medical improvement. A CA-7a is attached when the lost time is intermittent.

    How does travel reimbursement work?

Form OWCP-957A is used to request mileage reimbursement and OWCP-957B is used for expenses such as parking, tolls, or lodging related to authorized treatment. Reimbursement decisions are made by OWCP.

Can I choose my own OWCP doctor?

FECA generally allows an injured federal employee to select an initial treating physician, subject to applicable OWCP requirements. A later change of treating physician may require OWCP approval. Restore Muscle and Joint can provide information about our medical services if you are considering treatment with our providers.

How is treatment billed for federal employees?

For covered and authorized services related to an accepted FECA condition, Restore Muscle and Joint may bill the appropriate OWCP program directly. Coverage, authorization, and eligibility are determined by OWCP and remain subject to applicable program requirements.

Will your providers complete my OWCP forms?

Our clinical team completes the healthcare-provider portions of applicable OWCP forms related to your medical care, including the CA-16, CA-17, CA-20 and OWCP-5 series, and can explain the general purpose of the employee-completed forms. Call (816) 452-4488 and bring any paperwork you already have.

What if my claim was denied?

For patients whose claims have been denied or require additional development, our providers can evaluate whether additional medical examination, objective findings, diagnostic information, or medical-opinion documentation may be clinically appropriate. Appeal rights and claim decisions are handled through OWCP.

How long do I have to file a federal workers' compensation claim?

FECA generally provides a three-year period from the date of injury or from when the employee became aware the condition may be related to federal employment, with certain exceptions. Reporting promptly also helps preserve Continuation of Pay eligibility for traumatic injuries. See the Department of Labor at dol.gov for current requirements.
